Dear Student:

 The materials in this Introductory Packet for the Enhanced Placement (EP)
Program of Philadelphia Connections is being provided to you through your
school as an opportunity to enhance your field placement experience this year.

The enclosed literature and forms contain information that should help you
decide whether you would like to be part of the EP Program.  They include: 

  • Quick Start Instructions” – Just the basics of applying to the EP, in big print.  Useful as a checklist.
  • “What Is Connections?” – A description of this organization and its purpose;
  • EP Student Handbook – Covering all aspects of the program from the student’s perspective;
  • EP Student Agreement – A form that needs to be signed and return in order to participate;
  • EP Student Background Form – Complete to provide basic contact/background information
  • List of CBH-Funded Provider Agencies – The approved list of field placement agencies.
  • Two stipend invoice forms (one for first semester and one for second semester)

Your school must allow you to participate in the EP Program (check with your
placement administrator) and your placement agency or program must receive at
least some of its funding from Community Behavioral Health (CBH) for you to
participate in the EP Program.  Community Behavioral Health is the primary funder of
community/public behavioral health services for Philadelphia. 
To determine if an agency that you are considering is CBH-funded, refer to the
“CBH Providers List” in this packet
.

If, after reviewing the attached information, you wish to become part of the EP Program,
please inform the person who gave you this packet (if someone did) and fill out and mail
or fax
the Student Agreement and Student Background Form to Luciane Green,
Office Manager, as soon as possiblePromptness is important because admission to
the EP program is on a “first come-first served” basis.  You may contact
Ms. Green via fax, email, phone or post—see contact information in the Quick Start
Instructions. Ms. Green will send you any further information and instructions you need
at that point. 

 If you do not wish to join the EP program, there is no penalty of any sort, of course. 
However you will not be able to join later, after the school year has started.
