Philadelphia Connections SAMPLE Enhanced Placement Certification:
Letter of Understanding Between Provider, School, and Philadelphia Connections
The following understand and agree to fulfill to the best of their ability the actions or procedures listed under the appropriate sections below:
1. XYZ Provider organization or agency (hereafter referred to as PROVIDER); 2. School social work program of XYZ University, (hereafter referred to as SOCIAL WORK PROGRAM); 3. Philadelphia Connections (hereafter referred to as Connections).
No part of the conditions of this understanding should be altered without the agreement of Connections and/or the concurrence of all three parties (provider, school, and Connections). Definitions: Philadelphia
Connections is an organization funded
by the Department of Behavioral Health and Mental Retardation (DBHMR) which provides support for
collaborative efforts between schools of social work and care providers of the
Behavioral Health System (BHS) of Philadelphia.
The collaborative efforts described in this agreement are aimed at providing: · Enhanced field placement experiences to social work students who are in the School of Social Work program at SOCIAL WORK PROGRAM;
· Recognition, through EP Certification status, to PROVIDER for its efforts to serve as an outstanding placement site for BSW and/or MSW students and an enhanced educational and training environment for staff and students;
· Connections-funded presentations concerning important Behavioral Health System family/consumer issues for agency staff and school students and faculty;
· Connections-funded presentations on clinical supervision issues to PROVIDER staff who supervise students in field placements;
· The opportunity for both the school and provider agency to obtain additional funding for other collaborative efforts that benefit agency staff, school faculty and students, and, ultimately, BHS consumers and their families.
It is understood that the SOCIAL WORK PROGRAM will:
A. Designate one liaison person and a back-up to work with Connections staff; B. Provide Connections with contact information on all EP students and host agency liaison staff prior to the beginning of the field placement; C. Require the participation of the EP student in the Philadelphia Connections Enhanced Seminar Series (four Mondays spaced throughout the academic year), as well as two additional days of Connections-sponsored enhancement activities. The SOCIAL WORK PROGRAM placement liaison will assure students’ participation in EP activities in the same manner as with any other student placement obligation. D. Make arrangements with the assistance of Connections staff to host one family/consumer presentation at SOCIAL WORK PROGRAM no later than 5/31/07. SOCIAL WORK PROGRAM will choose the time, place and audience (staff and/or students at the agency) that will benefit from such a presentation. Arrangements should be completed by 1/31/07 for the presentation.
It is understood that PROVIDER will:
A. Designate one liaison person and a back-up to work with Connections staff;
B. Support the requirement that any PROVIDER Enhanced Placement program students participate in the Philadelphia Connections Enhanced Seminar Series on four field placement days as specified below. PROVIDER will also allow all EP students two additional days off at some time during their placement to attend other EP presentations unless special circumstances arise and are discussed with Connections staff in advance. The PROVIDER field placement supervisor will assure student participation in EP activities in the same manner as any other field placement obligations of the student.
C. Make arrangements with the assistance of Connections staff to host one family/consumer presentation at PROVIDER no later than 5/31/07. PROVIDER will choose the time, place and audience (staff and/or students at the agency) that will benefit from such a presentation. Arrangements should be completed by 1/31/07 for the presentation.
D. Provide time (when practical) for EP field placement supervisors to attend three to four Connections-arranged and sponsored presentations on clinical supervision issues during the course of the school year and summer. Connections will reimburse the host agency for any loss of billable time due to the staff member's absence, if necessary for the field supervisor to attend.
E. Inform Connections staff in advance, when possible, whenever there is a change in EP-related PROVIDER staff such as the field supervisor, and provide contact information concerning the new staff member(s).
F.
Attest, by signing
this Letter of Understanding, that PROVIDER adheres to standards developed by Community
Behavioral Health (CBH) and members of Connections regarding graduate students. These
A. Designate a staff member to work with SOCIAL WORK PROGRAM and PROVIDER liaison or backup, and provide full contact information for Connections staff liaison staff;
B. Provide comprehensive information to the EP students and their field supervisor(s) concerning Connections and the EP program. Connections staff will continue to provide this EP “orientation” to any new SOCIAL WORK PROGRAM or PROVIDER staff/faculty that become involved in the EP program as long as Connections is made aware of changes in staffing;
C. Schedule and manage the Enhanced Seminar Series including notifying all those involved in the EP what the schedule will be, what speakers are presenting on which topics, and presentation outlines and handouts when available.
D. Work with PROVIDER and SOCIAL WORK PROGRAM to arrange the family/consumer presentation each will host once during the time period specified above. This will include arranging for speakers provided by the Family Training and Advocacy Center for Serious Mental Illness (FTAC), reimbursing the presenters, taking responsibility for extra-agency publicity if the presentation is open to the public or other EP schools or providers, conducting audience evaluation of the presentation afterwards, and providing other administrative assistance as needed.
E. Design and conduct ongoing evaluation of the impact of the EP program and participants’ level of satisfaction and revise aspects of the program as needed. Results of this evaluation will be available to all EP participants.
F. Provide three to four Connections-arranged and sponsored presentations on clinical supervision issues for field supervisors during the course of the field placement. For presentations which take place during work hours, Connections will reimburse PROVIDER for the supervisors’ time at the rate of $30 an hour if desired.
G. Evaluate proposals for projects involving Connections funding and provide funds for appropriate proposals.
H. Provide the name of PROVIDER to area schools of social work and to appropriate BHS oversight agencies (including Community Behavioral Health) as an EP Certified Agency and provider of an enhanced educational environment for students and staff.
