QUICK START INSTRUCTIONS
For Provider EP Certification

FOR YOUR AGENCY TO BECOME EP CERTIFIED IN THE
PHILADELPHIA CONNECTIONS MSW ENHANCED PLACEMENT PROGRAM:

1. Read the materials in this packet -
contact Connections for clarifications;

2. Fill out the one-page Provider
Information form;
sign the Letter of Understanding
(Connections will make up a copy
with you agency's name and at least
one school whose students you have);

3. Fax or mail the two forms to Connections.

That’s all that’s involved. If, after being EP Certified you wish to apply for funding
for a project, please call Dr. Molinaro and he will help you through an equally simple
funding application process.
